California Residents
If you are a California resident, you have the right to request the following information relating to ComPsych's collection of your personal information in the 12-month period preceding your request: (1) the categories of personal information ComPsych has collected or disclosed about you; (2) the categories of sources from which your personal information is collected; (3) ComPsych's business or commercial purpose for collecting your personal information; (4) the categories of third parties with whom ComPsych shares personal information; and (5) the specific personal information ComPsych has collected about you. Please note that requests related to Personal Health Information (PHI) are subject to the Health Insurance Portability and Accountability Act (HIPAA) and, therefore, outside the scope of the CCPA. For requests subject to the CCPA, ComPsych will provide the requested information, free of charge, within 45 days of receiving your request. To make a request, you can contact ComPsych at 833-962-0005 or via email at Records@compsych.com. Further, you have the right to request that ComPsych delete any personal information about you that ComPsych has collected, subject to the limitations outlined in the California Consumer Privacy Act. ComPsych reserves the right to verify that any request for information or deletion originates from a California resident about whom the personal information pertains before disclosing or deleting the information. ComPsych also reserves the right to request information from a requestor in order to allow ComPsych to investigate and respond to a request pursuant to this paragraph.
You may have submitted personal information to ComPsych via ComPsych's web site or telephonically. In addition, your employer may submit information about you to ComPsych. Such information may include demographic and contact information and information related to your service request. This data may be used by ComPsych to (1) respond to you; (2) to provide services to you; (3) to allow ComPsych to manage its relationship with you, such as providing you with information it feels would be of interest to you or to facilitate responding to further requests or inquiries from you; (4) to personalize your Web site experience; and (5) to inform you of new products or changes to products and services. We will not intentionally disclose your personal information to third parties without your permission. However, personal information may be provided to third party service providers for processing purposes or in connection with a third party service provider's hosting or maintaining of this Web site.
ComPsych does not sell, and has not sold in the preceding 12 months, personal information it has collected to any third parties.
ComPsych does not discriminate against individuals on the basis of that individual's exercise of his or her rights pursuant to the California Consumer Privacy Act as described above.
